The cost of your first visit is $100, then $80 per treatment thereafter.

Please print out and complete each of the forms below, and bring the forms in with you on your first visit:

Patient Information and Agreement Form
Notice of Privacy Form

- Cash or check only
- All apointments or cancellations must be made by phone at 817-703-7601
- 24 hour notice is cancellation charges apply

Things to consider before arriving for treatment

• Make sure to eat something light an hour or two before arriving
• Try to avoid the intake of caffeine or pain medications
• Wear loose fitting clothes
• Bring a list of medications you are taking
